Cosmetic (Aesthtetic ) Oculoplastic Surgery

The eyelids are delicate structures protecting and maintaining the health of the eye. With the advancing age, the entire skin thins out due to loss of collagen especially in sun exposed areas, resulting in stretching and sagging.
The eyelid skin and underlying soft tissue are much thinner and elasticthan that in other parts of the face. On the other hand, the eyelids exposed to sunlight are the first place where this skin softening becomes obvious.Consequently, the eye region is one of the first areas to exhibit signs of aging.Besides sun damage, various additional factors, such as smoking, excessive alcohol consuming, obesity, systemic diseases such as diabetes, thyroid  ie. may cause skin, muscles  and underlying supporting tissue to weaken further and earlier.

The contribution of eyelids to face cosmetic and beauty is unquestionable.

If the eyelid skin begins sagging beyond eyelid margin into the field of vision, the sagged tissue  may interfere with vision affecting the patient’s superior field of vision. Heavy upper eyelids due to sagging tissues may cause the patient looking droopy to frown the forehead constantly. The sagged eyelids affecting peripheral vision can make some daily activities, such as driving, more difficult.In some cases, the brows together or not with upper eyelid skin can be drooped. This condition should not be overlooked. In these cases, the brows should be lifted at desired position before all. Any upper eyelid blepharoplasty surgery alone without a “brow lift” may detereiorate the situation by making  closer the brow to eyelids. The brow lift can be performed by external approach or internally using endoscopic surgery (internal browpexy).
The same involutional changes, loss of elasticity of the skin and sagging, may also cause lower eyelid ‘bags’, and cause protrusion of fat. The lower eyelid changes may cause a “tired look”, further,  as if an ‘alcohol addict’ appearance.
Either upper and lower eyelid cosmetic surgery, called blepharoplasty,  can rejuvenate  appearance. It is a common, safe and effective cosmetic surgical procedure when done by an experienced “Oculoplastic Surgeon”. A customized surgical approach could also rectify asymmetry,if any,  and achieve a nice aesthetic outcome.
After surgery, with satisfactory results, self confidence may increase and people may feel more peaceful with him/herself
The aim of cosmetic eyelid surgery (blepharoplasty) to remove excess skin or fat even muscles from the eyelids and get a more fresh and rejuvenated youthful appearance. It also eliminates the drooping of skin into the visual field, greatly improving peripheral vision and eye fatigue.
Blepharoplasty can be performed on either the upper or lower eyelid, or on both, in one session usually under local anesthesia with sedoanalgesia. 
It is important for a patient to have realistic expectations before undergoing cosmetic blepharoplasty. Although the procedure can enhance appearance and improve self-confidence, it does not radically alter the whole face. 
An eye lift will not eliminate dark circles under the eyes, crow's feet, or other facial wrinkles. It is often done along with other procedures such as laser resurfacing, filler or botox injections.
Healthy individuals without any systemic disease or any eye disases that can impair the longterm surgical prognosis, non-smokers, non heavy alcohol-consumers, individuals at peace with a positive outlook and realistic goals are good candidates for a cosmetic eyelid surgery.
In some cases, due to extensive facial changes, a full or mini facial lifting surgery may be required in conjunction with a brow lift and eyelid surgery to rejuvenate aging face as well as eyes. 
Also, an adjunctive treatment to surgery such as fillers, botox, laser resurfacing ie. may be offered to improve the quality and texture of the skin either before or, usually, after an eyelid surgery.
A number of different types of surgeons can perform cosmetic blepharoplasty. Ophthalmologybased oculoplastic surgeons (a surgical sub-specialty of ophthalmology are more acquainted with eye and surrounding structures. So, They, most commonly, perform these types of procedures. However, general plastic surgeons also perform cosmetic eyelid surgery.
After an uneventful surgery, temporary discomfort, feeling of “tightness” in the eyelid area, swelling or bruising may be seen. Excessive tearing and sensitivity to light for the first few weeks should be expected. Temporary dryness, burning, or itching of eyes may also occur. Wearing dark glasses may be helpful during this period. All these side effects disappear in weeks.
It should be remembered that a cosmetic blepharoplasty is not a complication free procedure.
